Checklist of Documentary Requirements for Scholarship
RENEWAL APPLICATION
AY 2014-2015 (For High School Students)
2nd Semester, 2014-2015 (For College Students)
- Renewal Application Form
- Liquidation report for the scholarship received (Previous Semester/School Year)
- School’s assessment of fees ( 2nd Sem, SY 2013-2014/School Year)
- Evidence of scholarship received from the school or from any other institutions/agencies
- Official Receipts/proofs of payment of tuition, enrolment and other related fees & expenses
- New Student ID (if applicable)
- Certificate of Registration
- Official Receipts for rental (boarding house/dormitory, etc.)
- Report of Ratings with GWA (last semester; must be from a school record)
NEW APPLICATION
- Application Form
- 2 x 2 picture
- Birth Certificate
- Latest Student ID
- Transcript of Records
- Certificate/s of Recognition/s
- Documentary evidence of achievements activities/organization involvements
- School’s assessment of fees
- Evidence of scholarship received from the school or from any other institutions/agencies
- Official Receipts/proofs of payment of tuition, enrolment and other related fees & expenses
- New Student ID (if applicable)
- Certificate of Registration
- Official Receipts for rental (boarding house/dormitory, etc.)
- Any of the following: Latest Income Tax Return (ITR) of parents or guardians, Certificate of tax Exemption from the Bureau of Internal Revenue (BIR) or Affidavit of No income
- Certificate of Indigency
- Certificate of good moral character from the last school attended
