*Renewal Application Form
*Liquidation Repoort for the scholarship received (Previous Semester / School Year)
*School Assessment of fees
*Evidence of scholarship received from the school or from any other institutions / agencies
*Official receipts / proofs of payment of tuition, enrolment and other related fees & expenses
*New Student ID
*Certificate of Registration
*Official Receipts for rental (boarding house / dormitory, etc.)
*Report of Ratings with GWA (last semester, must be from a school record)
